Payment Information
Unpaid balances must be paid in full by Monday, May 19, 2008. If you fail to do so, your registration may be subject to cancellation. All forms of payment must be in US dollars. If your organization is tax exempt, the University of Wisconsin tax identification number is: 39-1805963.
CREDIT CARDS: Participants paying via credit card must submit payment of their registration fee at the time of registration. To make additional payments, please go to the Payment Link.
PURCHASE ORDERS: Participants paying via an institutional Purchase Order will receive an invoice, which they should forward to the appropriate financial personnel for prompt payment no later than Monday, May 19, 2008. Checks can be made payable to: The Wisconsin Union-Zebrafish2008. Personal checks are not accepted.
Cancellations
Wisconsin Union Conference Services must receive cancellation notification in writing (zebrafish@union.wisc.edu) no later than Monday, May 19th. After this date, all payments are NON-REFUNDABLE. If cancellation notification is received on or before Monday, May 19th, the registration fee is refundable, minus a $100.00 handling fee.
